Pre-visit forms
Collect history, medications, allergies, and files before the visit, and read the brief.
Pre-visit forms collect a patient's information before the appointment, so the doctor starts the visit with a short brief instead of a blank page. Patients fill them in on their phone, in Arabic or English, without an account.
What patients are asked
A form is built from sections you can show, hide, or make required:
- Reason for visit and symptoms, in the patient's own words
- Medical history, medications, allergies, and previous surgeries
- Lab results and reports (photos or PDFs up to 15 MB each)
- Insurance and referral details
- Your own clinic questions
- Consent, signed by typing their full name
- Arrival instructions
When the clinic already has a list on file, such as medications, the patient sees it and confirms what is still correct instead of typing it again.
How forms reach patients
- Automatically: a form set to Send automatically when an appointment is booked goes out with the booking confirmation.
- From the Pre-visit page: choose Send form, Remind, or Copy link for any upcoming appointment.
- From the appointment: choose Send pre-visit form in its brief.
Fatma sends reminders on WhatsApp to patients who have not finished, and confirms when a form is submitted.
To protect patient information, the link asks for the patient's date of birth or the last four digits of their phone number before it opens. Too many wrong attempts lock the link for 30 minutes. Links expire after the appointment time, and stop working if the appointment is cancelled.
Track who is ready
Open Pre-visit (/pre-visit) to see upcoming appointments for the next 7 days with each form's status: Not sent, Not started, In progress, Completed, Expired, or Cancelled, plus what is still missing.
The Pre-Visit Brief
The brief appears on the appointment and inside the visit workspace. It shows:
- completion and when the form was submitted or last opened;
- anything required that is still missing;
- the patient's answers and uploaded files;
- Changes since the chart: items that are new or no longer listed compared with the patient's file.
After reviewing the lists with the patient, choose Update chart with confirmed lists. Only lists the patient confirmed are copied.
Create your own forms
On the Pre-visit page, open Forms and choose New form. Give it a name, then optionally limit it to a department or an appointment type. Turn sections on or off, mark them required, and add clinic questions in English and Arabic with short text, long text, yes or no, choice, or date answers.
If several forms match an appointment, the most specific active one is used: department and type first, then type, then department, then any. Until you create a form, patients get the default form.
